Heritage YMCA Group Porpoise Swim Team ISI Form Page updated on 7/26/2010 The Porpoises are proud to be an ISI chartered swim team that will be participating as a team at ISI meets. To be included in the ISI meets on the Porpoise Meet Schedule, you will need to download, print, fill out and return to either Coach Bob, or Erica Kroll, the ISI Athlete Registration form (click the download link) and a $60 ISI/USAS membership (insurance) fee check (made out to "Heritage YMCA" with the note "ISI Reg." in the memo area of the check). Families can combine fees into one check (2x$60=$120), but each swimmer needs an INDIVIDUALLY completed form. Forms and checks must go to Erica or Coach Bob. ISI meet participation and registration is an OPTION, not a requirement. ISI Form Forms MUST be filled out and given to Erica for processing.  Erica MUST have your form and check before you can be registered. Any swimmers who sent their form directly to ISI in Des Plaines, must provide a photocopy of the ISI card.  Make checks for the US Swimming (ISI) to "Heritage YMCA Group", not to ISI directly.
